I. OBJECTIVE

1. To provide guidelines for reporting of absences

II. DEFINITION OF TERMS

1. Absence – the inability to report for work on a regular working holiday.

III. GENERAL POLICIES

In the event that you are unable to come to work, be sure to call your immediate superior and HRD at least two (2) hours before your duty. For BBT/CDH/SSM/WDF/FS personnel, notify Store Operations Support at least two (2) hours before your duty, so that arrangements for immediate assistance and/or substitution shall be made. A leave of absence must be scheduled at least forty-eight (48) hours in advance except in cases of emergency.

Absence will be considered permissible only through the following reasons:
• Any form of sickness should be accompanied by a letter or certificate from a qualified doctor. It is important that a letterhead is used with the accompanying license number. The medicines prescribed should also be written on a similar letterhead.

• For any sickness which does not require medical attention of a qualified doctor, a formal letter from the employee’s guardian should be presented before coming back to work.

• Bereavement leave (death of an immediate family member), should be notified by a letter coming from the employee’s guardian.

• For emergency situations such as calamities or fortuitous events like earthquakes, tornadoes, typhoon, flash floods, landslide, vehicular and major accidents:
The employee is still obliged to call his immediate superior unless a verifiable emergency makes it impossible for the employee to do so; hence, any call from the immediate family is sufficient within forty-eight (48) hours.

• Employees who will file a leave of absence cannot use the “personal problem and/or emergency” and other generic reasons as a justification for their absence. They should inform HRAD with the specific personal reason.
The company shall only consider the aforementioned reason as valid if with prior approval by immediate superior and HRAD and duly signed in the Leave of Absence form.

• Absence of three (3) or more consecutive scheduled working days without notifying the immediate superior will be considered Absence without Official Leave (AWOL) and may result in dismissal/termination of employment.
