I. OBJECTIVE
1. To introduce the other kinds of forms that the outlets use aside from the set of DSR
2. To define the role of the SOS in providing each outlet with the necessary forms for their daily operations
II. DEFINITION OF TERMS
1. Authority to Deduct (ATD) Slip – the form used in issuing monetary charges and acknowledging it
2. Cash Pull-Out Slip – the form used to pull-out the sales or part of the sales of an outlet
3. Claim Change Form – the form used during incidents when the customer forgot his or her change and retrieves it the next day
4. Incident Report Slip – the form used to state the details of an incident
5. Order Slip – the form used for every single transaction where the Service Crew lists the order of the customer. A claim stub at the bottom part of the order slip serves as the proof of the customer for his/her order (for BBE, BBT, CDH, and WDF outlets)
6. Overring Slip – the form used to declare variance, specifically over, in cash on hand during audit
7. Pull-Out Receipt – the form used when a pull-out of item is to be made
8. Requisition Slip – also known as “RS”, is the form used by the requesting party in making any kind of item or service request
9. Senior Citizen Discount Form – the form used for every transaction with a senior citizen to declare the amount of discount of the customer
10. Transfer Slip – the form used when a stock or item must be transferred from one outlet to another
III. GENERAL POLICIES
The following forms are labeled as “other forms” as they are not included in the set of forms that the outlet must regularly use and send every operating day. These “other forms” are merely accessories and used only as needed:
• Authority to Deduct Slip
• Cash Pull-Out Slip
• Claim Change Form
• Incident Report Slip
• Order Slip (BBE, BBT, CDH, WD)
• Overring Slip
• Pull-Out Receipt
• Requisition Slip
• Senior Citizen Discount Form

These other forms must be requested by the outlet. SOS does not have responsibility of issuing these forms without a request from the outlet. It is the responsibility of the Service Crews of the outlet to ensure availability of these forms in their outlet. (For the process of requisition, see document on Requests.)
