1. To provide guidelines on the process of securing work permit for contractor works inside the company premises

III. GENERAL POLICIES

All departments who will be requesting for a contractor who will work inside the company premises must submit a “Work Permit Letter”. The work permit must have the following details addressed to HRAD:
• Details of work
• List of Personnel
• Tools and Equipment
• Name of Contractor
• Duration
• Site Location

The work permit must be noted by COO and approved by CEO two (2) days before the start of target date. Approved work permit must be submitted to HRAD. HRAD will provide a copy to guard on duty for security reasons.

Guard on duty must check the contractor’s employees’ ID, equipments, bags and other personal belongings before and after working hours. All contractors are subject for body frisking and inspection. All contractors are not allowed to bring equipments, garbage and other paraphernalia outside the company premises most especially if not found in the list of tools and equipment in the work permit letter. Contractors must secure a gate pass to HRAD for them to be able to bring their own equipments outside the company premises.

Guard on duty must immediately report to HRAD any non-compliance with regards to the rules and regulations.

II. DEFINITION OF TERMS

III. GENERAL POLICIES

SOS must prepare Work Permit for maintenance or repair works that need to be done in the outlet. Work permit must be submitted two (2) days prior to the scheduled day of work. Each permit must be limited to two (2) hours of work to avoid any unnecessary charges. Attached to the Work Permit is a formal letter addressed to the mall manager or administrator explaining the need for the permit. Scheduling of work permit must be coordinated with the Team Leader to avoid any conflict during store operation.

Special cases:
• Work permit for SM Megamall should have 3 days lead time prior the work to be done; while work permit for SM Fairview and SM North Edsa should have 1-2 days lead time.
• Work permit for SM North Edsa – Food Court should be scheduled from 8:00am-9:00am or 9:30pm-10:00pm only. Work permit scheduled during mall operation hours or 10:00am-9:30pm and after mall operation hours or from 10:00pm onwards has penalty charges.
• All SM Work permits should be submitted to the admin office before 12:00nn and must be in five (5) printed copies.
